The schemes

Three schemes, one network of hospitals

Residents of Jammu & Kashmir may be covered under PM-JAY, under the Union Territory’s SEHAT extension, or under the Vay Vandana programme for senior citizens. All three are honoured at the same empanelled hospitals.

An extension of PM-JAY that provides universal health coverage to all residents of the Union Territory, irrespective of inclusion in the SECC 2011 database.

Ayushman Vay Vandana is a national programme for citizens aged 70 and above. It is separate from the State-level SEHAT card, which may carry different eligibility conditions. If you are unsure which applies to you, call 104.

Getting your card

Four steps to your Golden Card

Card generation is free at every stage. An operator will never ask you for a fee, and you do not need a middleman.

  1. Visit an operator

    Go to your nearest Arogya Mitra desk, Common Service Centre or empanelled hospital help desk in Pulwama.

  2. Provide your details

    Share your Family ID, ration card number or the mobile number registered against your family record.

  3. Complete verification

    The operator verifies your record against the beneficiary database and captures the beneficiary photograph.

  4. Receive your card

    Your digital Golden Card is generated and can be downloaded immediately. No fee is payable at any stage.

Documents to carry

  • Aadhaar card of the beneficiary
  • Ration card or Family ID
  • Any government-issued photo identity document
  • Mobile number for verification

Eligibility

Three official ways to check whether you are covered

Eligibility is determined by the National Health Authority from the beneficiary database. Use any of the channels below for a verified answer, and do not share your Aadhaar or card details with unofficial websites or agents.

  • On the NHA beneficiary portal

    Search by mobile number, Aadhaar, ration card or Family ID, and download your card once verified.

    Call the district helpline on 104, or the National Health Authority helpline on 14555, for assistance in checking your record.

    Visit District Hospital Pulwama, SDH Tral or your nearest empanelled hospital with your ration card and Aadhaar.

  • Ayushman Bharat PM-JAY is the world's largest health assurance scheme. It provides cover of ₹5 lakh per family per year for secondary and tertiary care hospitalisation at empanelled hospitals.

  • Check on the National Health Authority beneficiary portal at beneficiary.nha.gov.in, call the helpline on 14555 or 104, or visit an Arogya Mitra desk with your ration card and Aadhaar.

  • Carry your Aadhaar card, ration card or Family ID, and any government-issued photo identity document. The operator verifies your details against the beneficiary database.

  • No. The Golden Card is issued free of cost, and treatment at empanelled hospitals for covered procedures is cashless. If anyone demands payment, lodge a grievance through CGRMS or call 104.

  • Yes. The card is portable across India. You can receive treatment at any empanelled hospital in any State or Union Territory.

  • The Golden Card is issued under PM-JAY to families listed in the SECC 2011 database. The SEHAT card extends the same ₹5 lakh cover to all residents of Jammu & Kashmir under the AB PM-JAY SEHAT scheme.

  • Contact the Arogya Mitra desk at the hospital first. If the matter is not resolved, call 104 or the district scheme office, or lodge a formal complaint on the CGRMS grievance portal.
