Visit an operator
Go to your nearest Arogya Mitra desk, Common Service Centre or empanelled hospital help desk in Pulwama.
Provide your details
Share your Family ID, ration card number or the mobile number registered against your family record.
Complete verification
The operator verifies your record against the beneficiary database and captures the beneficiary photograph.
Receive your card
Your digital Golden Card is generated and can be downloaded immediately. No fee is payable at any stage.
Documents to carry
- Aadhaar card of the beneficiary
- Ration card or Family ID
- Any government-issued photo identity document
- Mobile number for verification
Zero cost. Both the card and covered treatment are free. Report any demand for money on 104.
